Waiver
of Liability
This
form is required for all participants wishing to
climb ‘The
Rock’ at the Morris Recreation Center. This
waiver is good for one year only and must be updated
annually.
Ground
School Check-Off
This
form is required for all participants who wish
to receive ‘belayer’ certification at ‘The
Rock’ at the Morris Recreation Center. Ground
School certification is valid for one semester
only; please check with the Outdoor Adventure staff
about recertification.
Assumption
of Risk
This form is
required of all participants registered for an
Outdoor Adventure trip.
Emergency
Information
This form
is required of all participants registered for
an Outdoor Adventure trip.
Gear Rental
This form is
