the Purchasing Card
Obtaining a Purchasing Card
If you have Adobe Reader 4.0 or higher, click on the Cardholder
Application/Approval link under #1 to the right. Your Department Head or
Account Manager must approve and forward to the Purchasing Department.
If you do not have Adobe Reader 4.0 or higher, please click on the
following link to download a copy.

Once you have obtained you copy of Adobe Reader, then proceed by
completing the Cardholder Application/Approval form under #1. Your
Department Head or Account Manager must approve and forward to the
Purchasing Department.

How to Get a Purchasing Card
- Complete and submit a Cardholder
Application/Approval form to the Program
Coordinator:

This application will provide the Purchasing
Department with
necessary information about you (the cardholder), the
budgeted account/support account for default payment,
and the Department Head's approval of your
application.
- Program Coordinator will request issuance of the
Purchasing Card from Bank of America.
- Program Coordinator will contact the card applicant
after their card arrives to schedule a Purchasing Card
training session. Applicants must bring a photo
I.D. to the training session for verification and
identification. (Training is waived for those who
have already attended a session.)
- Applicant attends training session (approximately
1.5 hours) and signs the Cardholder Purchasing Card
Agreement affirming that the cardholder has read and
understands the policies, procedures, and
responsibilities associated with the Purchasing Card.
(If you have already attended a session, you may pick
up your card from the office
of the Program Coordinator.)
- Card is issued.
Activating your Purchasing Card

The cardholder must call 1-800-535-6807 to activate the
Purchasing Card before using it. Upon receipt of the card,
the cardholder should sign the back of the card to
complete the activation process.
