Employee Training and Development  

Training Authorization Request Instructions

The training course must be approved via a “Training Authorization Form”.  This form must be sent to HR for final approval after all signatures are obtained.  The Training Authorization Form should be submitted at least 2 weeks before the 1st workshop.  You will be sent the 2nd and 3rd copies of the authorization form, a letter of authorization, sign-in sheets for each session and evaluation sheets.  

The trainer must state times, dates, number of people allowed, and who the targeted audience is, and if you want it posted on the web (See HR Training Program).  If you do not want it posted on the web, please state clearly on form (DO NOT POST).  If you want certificates – state clearly on form - want certificates.

Trainers please do the following:

  1. secure the presenter,

  2. schedule the meeting room and media equipment,

  3. ensure that participants sign-in on appropriate sheets and evaluations are completed  (these will be furnished to you by Human Resources), 

  4. after the session, please forward sign-in sheets and evaluation instruments  to the HR Office, Social Science Building Room 137 for filing, and

  5. if the training is canceled, please mark "canceled" on the authorization form with the required signature of the contact person.

Before the training session, The Human Resource Department will:

  1. notify targeted University employees of the training session,

  2. place training on University-wide training calendar, Note:  write yes or no on form to post on web (See HR Training Program).  

  3. furnish sign-in sheets and evaluation instruments for completion by participants.

Following the training session, The Human Resources Department will:

  1. award a certificate to attendees or furnish an acknowledgment of attendance to supervisors (if requested),

  2. place acknowledgment in employees’ personnel files, and

  3. maintain a list of participants.
