Termination Procedures

Checklist - Upon termination notice, an employee should come by the Human Resource Office for closure on:

1. Vacation and Sick - How much vacation time remaining and donation to the Sick Leave Pool
2. Retirement - Teacher Retirement
3. Insurance - COBRA

Exit Interview Questionnaire - This form is optional for employees to complete and return at the time of his/her termination.

Property Clearance Verification - Supervisors are required to complete this form upon termination of departmental employees. This form should be forwarded to Human Resources.
