Eligibility Requirements and Instructions for completing Hazlewood Tuition Exemption Form HE-P Eligibility Requirements and Documentation In order to qualify for a tuition and partial fee exemption through the Texas Education Code 54.203 (known as the Hazlewood Act), a person must meet all program requirements. The following is a list of program requirements and how they are documented.
Instructions for Completing the Application for Previous Exemption Recipients. Questions 1-3 Self-explanatory Questions 4 This is just to help the Hazlewood Administrator know which documents should be on file. If you are a dependent child of a deceased or disabled member of the Armed Forces, please give the name and social security number of the member. Question 5 If you have never used the Hazlewood Exemption you should not be completing this form. If you are a veteran and have never used Hazlewood, you should be using form HE-V. If you are a dependent and have never used Hazlewood, you should be using form HE-D. Question 6. Since your eligibility for the exemption lasts for 150 hours, your current institution may need to contact your previous school to make sure it has an accurate count of your remaining hours of eligibility. Question 7. Persons who are in default on a loan made or guaranteed by the federal government or state of Texas may not use the exemption. If you answer “yes” to this question, you cannot use the exemption. Your institution will verify your response to this question. Question 8. To use the exemption, a veteran’s or dependent’s federal veterans’ education benefits cannot exceed the potential value of his/her Hazlewood exemption (tuition and fees other than property deposit and student service fees). If he/she still has federal veterans’ education benefits available, he/she must use them first or while using the Hazlewood exemption. As a continuing user of the exemption, you already have a letter on file. Question 9. (For veterans only; not for dependents.) If you have served active duty since the issuance of your DD214 you may be eligible for additional federal veterans’ benefits. You need to contact Muskogee Oklahoma to acquire a current VA benefit letter. They can be reached at 1-888-4424551. Documents to Submit. To apply for a Hazlewood Exemption, you must submit the following documents to your institution: 1 The Hazlewood Exemption Application for Previous Recipients 2 Benefits letter from Muskogee, Oklahoma – only required of veterans who have served additional active duty since their DD214 was issued. Used to verify that you have used up your federal veterans education benefits. |
